Grade I  –  enlargement of the glands (small breast)

Grade II – enlargement of the glands and accumulation of fats around the chest (moderate breast)

Surgery for Gynecomastia Grade 3 is divided into three parts (1) Liposuction (2) Gland Removal and (3) Skin Resection (Areolar Type). Among the three, the part that is most crucial and concerning for the patients is the “skin resection”. This is because of the scar it will leave after the surgery.

The necessity to receive the Areolar skin resection depends on the patient. It does have its advantages. The purpose of this procedure is to remove the excess saggy skin which has been stretched and enlarged due to the glands and fats. Choosing to include this procedure, would give patients a flatter and normal-looking chest.

Anyone with this Grade can have this surgery. But for our younger patients, specifically 40 years old and below and single, we advise them to proceed with the liposuction and gland removal first. This is because they still have the chance to improve the condition of their skin sagging by exercising really hard after the surgery and improving their overall health.

Grade 2 Gynecomastia cases: Both patients above are in their 20s. Looking at their “Fitness Score”, the one below, has a higher BMI. Their glands are almost of the same size. But you can clearly see the difference of their results 4 weeks after their surgery. The one with a good Fitness Score has a better-looking chest. If one neglects to exercise after their surgery, the result will be of that of a “crying chest”.

Whether one decides to have the skin resection or not, working out after the surgery is very important.
It is hard, we know.

Sometimes there are fatty proliferation of the breasts, without proliferation of glandular tissue.
It is very difficult to distinguish from normal male breast on mammography. It can be diagnosis requires clinical correlation with breast enlargement.
Normal gynecomastia refers to the overgrowth of fat tissue and mammary gland tissue. Pseudogynecomastia refers to the state of mostly fat tissue, rather than mammary gland tissue.

As mentioned above, the true gynecomastia and the pseudogynecomastia differ greatly in their cause. In case of false gynecomastia, the fat is caused by the growth of obesity, etc, But in case of true gynecomastia it is the status which mammary gland is overgrowth.

In case of pseudogynecomastia, if there is little or no development of the mammary gland, and only the fat tissue is overdeveloped, symptoms can be improved by exercise or diet.

However, in the case of true gynecomastia, the symptoms cannot be improved by a diet. Developed mammary tissue does not disappear naturally by exercise. In this case, you will be treated surgically.

It could be classified acording to which parts is enlarged : (1) True Gynecomastia – gland enlargement (2) False Gynecomastia – Over fat accumulation (3) Mixed Type – Both gland & fat over growth.

And, It could also be classified according to its time of onset & cause,

– Newborn gynecomastia

  • A physiological response to high levels of maternal and placental estrogen transferred in utero. A secretion sometimes called witch’s milk can often be expressed.

– Pubertal gynecomastia:

  • A physiological response to the increase in testosterone fueled by marked increases in GH, insulin-like growth factor-1, FSH, and LH at puberty
  • Estrogen increases 3-fold, but peaks earlier than testosterone.

– Adult gynecomastia (physiological):

  • A physiological response to the decrease in free testosterone and the increase in adipose tissue that often accompanies ageing
  • Testosterone is converted to oestrogen in adipose tissue.

– Adult gynecomastia (non-physiological):

  • A response to increased oestrogen effect relative to androgen effect from a cause other than ageing.

– Pseudo-gynecomastia:

  • Male breast enlargement entirely due to adipose tissue.
