Grade I  –  enlargement of the glands (small breast)

Grade II – enlargement of the glands and accumulation of fats around the chest (moderate breast)

Surgery for Gynecomastia Grade 3 is divided into three parts (1) Liposuction (2) Gland Removal and (3) Skin Resection (Areolar Type). Among the three, the part that is most crucial and concerning for the patients is the “skin resection”. This is because of the scar it will leave after the surgery.

The necessity to receive the Areolar skin resection depends on the patient. It does have its advantages. The purpose of this procedure is to remove the excess saggy skin which has been stretched and enlarged due to the glands and fats. Choosing to include this procedure, would give patients a flatter and normal-looking chest.

Anyone with this Grade can have this surgery. But for our younger patients, specifically 40 years old and below and single, we advise them to proceed with the liposuction and gland removal first. This is because they still have the chance to improve the condition of their skin sagging by exercising really hard after the surgery and improving their overall health.

Grade 2 Gynecomastia cases: Both patients above are in their 20s. Looking at their “Fitness Score”, the one below, has a higher BMI. Their glands are almost of the same size. But you can clearly see the difference of their results 4 weeks after their surgery. The one with a good Fitness Score has a better-looking chest. If one neglects to exercise after their surgery, the result will be of that of a “crying chest”.

Whether one decides to have the skin resection or not, working out after the surgery is very important.
It is hard, we know.
But the results will be worth it.

3단계이상의 여성형 유방증에서는 항상 피부절제를 필요로 하는가?

앞서 여성형 유방증의 단계에 따른 수술방법을 설명하였습니다.

이러한 수술방법은 수술 후의 결과를 예측하고, 가장 이상적인 체형을 만들기 위한 방법입니다.

즉, 아래와 같은 여성형 유방증의 단계에서 피부의 쳐짐 정도가 중요하며, 그 쳐짐 정동에 따라서, 단순 유선제거나 지방흡입술 후에도

피부가 늘어진 주머니와 같이 쳐지는 것을 최소한으로 하기 위한 것입니다.

 

따라서, 에비타흉부외과에서는 3단계 이상부터는 다양한 방법의 피부절제술을 통해서, 유방거상술(Mastopexy)을 시행하는 데,

이것이 꼭,, 정답은 아닙니다.

동일하게 20대 초반의 2단계 여성형 유방증 환자입니다.

비슷한 양의 유선과 지방을 제거하였지만, 1개월 후의 그 결과는 차이가 있습니다.

그러한 차이의 이유는 비만도, 피부탄력도, 선천적 체형 등의 여러 이유가 있지만,

가장 중요하게 생각하는 것이 수술 후의 운동입니다.

사진에서 잘 보면, 수술 후 운동으로 인해서 복근의 변화도 대략 볼 수 있습니다.

수술 후의 피부쳐짐을 최소화하기 위해서, 근막거상, 아큐스컬프 레이저, 수술 후 고주파 관리 등을 시행하고 있지만,

가장 중요한 부분은 수술 후 환자 본인의 의지인 것입니다.

 

그런면에서 3단계 이상의 여성형 유방증의 경우에도 에비타흉부외과에서는 3가지 사항을 고려하고,

피부절개술을 할지를 결정하는 것입니다.

 

첫째는 환자의 나이입니다.

피부절개술은 어떤 크기나 방향이든지 흉터가 남게 됩니다.

이러한 흉터는 수술 후의 만족도를 떨어뜨리는 큰 요소입니다.

따라서, 30대 미만의 나이인 경우에는 비록 3단계나 4단계의 여성형유방증인 경우에도

가능한 피부절개술을 하는 것을 신중하게 고려하게 됩니다.

그 이유는 쉽게 예를 들자면, 우리가 수영장이나 바닷가 등에서 레저활동을 할때,

10대나 20대의 경우에는 상체를 노출시키려고 합니다.

하지만,

하지만, 40대가 넘으면, 대개 T-Shirt나 래쉬가드를 입게 됩니다.

따라서, 흉터가 좀 남아도 신경을 덜 쓰게 되는 것입니다.

 

물론, 가슴이나 배, 심지어는 흉터도 전혀 신경쓰지 않는 분들도 있습니다.

 

따라서, 30대 미만의 경우에는 가능한 유륜옆의 최소절개로 흉터를 최소화해서 지방흡입과 유선제거만 하고,

수술후 관리나 운동으로 한번더 기회를 가져보는 것도 바람직합니다.

 

 

둘째는, 운동성향, Fitness Score를 고려합니다.

사실상, 운동과 근력강화를 키우는 것은 개인적인 성향입니다.

운동을 좋아하는 성향이 있고, 운동보다는 독서나 음악감상 등을 좋아하는 성향이 있습니다.

이러한 성향은 쉽게 바꾸기가 어렵습니다.

이러한 부분의 판단은 수술전 시행하는 인바디(Body Composition Analysis)에서 Fitness Score을 참고합니다.

이 Fitness Score가 낮은 경우, 운동이나 근력강화를 좋아하는 성향이 아니라고 판단을 하고,

수술후에 개인 피트니스나 트레이닝, 헬스장 등록 등을 권장하기도 합니다.

 

세째는 개인의 의지입니다.

수술전에 이러한 사항을 설명하면, 대부분 운동과 근력강화를 열심히 하겠다고 합니다.

하지만, 이는 작심삼일이 되는 경우를 숱하게 보았습니다.

 

영화 “300”에 출연한 연기자들처럼, 단기간에 몸을 만드는 정도는 아니지만,

그래도, 지방과 유선이 제거된 후에 남은 피부를 대흉근으로 대체를 하기 위한 어느정도의 노력이 필요합니다.

이 부분에 있어서, 중요한 부분이 개인의 의지입니다.

이러한 의지가 바쳐줄 때, 여성형 유방증 수술 후의 바뀐 체형은 더욱 만족을 줄 것입니다.

There are many factors that could lead to the appearance of these kinds of scars. One must understand that factors like genetics, skin type and previous history of formation of Hypertrophic scars and Keloids are natural and unmodifiable.

Keloids usually develops after the healing of incision or skin injury. It is the result of overgrowth of scar tissues. It is characterized by raised scar that grows much larger than the original would or incision. The color could go from white, red to the color of your skin.

Hypertrophic scar is the most common among the two. It is not harmful to one’s health but it can be bothersome.  People receive treatments mostly for cosmetic reasons. It is characterized by thick and raised scar that does not go beyond the original scar.

Hypertropic scars usually heal on its own overtime. Keloids, on the other hand, can continue to grow. More often than not, keloids need treatment to effectively shrunk it or remove it.

1.Steroid Injection

This is the first line of treatment for keloids and hypertrophic scars. This is done once every 3 weeks. This treatment would often respond immediately to hypertropic scars. For keloids, it would require a repeat treatment but still with high rate of recurrence.

Silicone Sheet and Gel

Silicone sheets are soft, self-adhesive sheet that can be applied on the scar. It can help reduce the thickness of the scar and scar color. It should be worn at least 12 Hours a day. This will reduce collagen production resulting to flatter scar. Applying silicone sheets can also prevent the area from bacterial invasion that can induce excessive production of collagen. And this serves very useful in reducing the itchiness and discomfort associated with the scar.

Silicone gel is a recent treatment and serves as a more effective alternative to Silicone sheets. It works 24hours a day and dries easily after 4-5minutes. The advantage of using this treatment is that it is easy to use and it proves to be effective even for children and with persons who has sensitive skin.  This is especially convenient for bigger and regular scars that don’t fit on sheets.

Radiation Therapy and Surgical Removal

These procedures are considered the last treatment and most effective plan for reducing resistant keloids.  It can greatly improve the appearance of the scar but it will not entirely remove it. The rate of recurrence is still high especially for keloid-prone patients.

The procedure will only take 10-30minutes depending on the size. This is done through Local Anesthesia.

에비타흉부외과의원에서도 “여성형유방증”에 대한 치료 비용의 변경 있습니다.

1단계 –  국민건강보험 적용 제외

: 비급여 시술비용은 유선만 게거하는 경우와 지방흡입을 동반하는 경우엔 따라 비용의 차이가 있습니다.

(대략 100만원 ~ 200만원 선)

2단계 – 유선제거 + 지방흡입술 / 국민건강보험 적용

: 유선제거 등이 보험이 적용되고, 지방흡입술은 청구나 수납이 되지 않습니다.

압박복, 초음파 검사비,  특수 수술재료, 입원료 등에 따라 차이가 있으며,

환자부담금은 대략 80만원 ~ 110만원가량이 됩니다.

3단계 이상 – 일부 국민건강보험 적용

: 유선제거는 국민건강보험이 적용되고, 지방흡입술은 비청구 대상이지만,

유륜축소나 피부절제술, 유방거상술, 유두성형술 등은 미용상의 비급여 시술이므로 추가 비용이 발생합니다.
